You really don't even need the car to be running and 3000rpm is a bit excessive. What I was referring to is watching the belt as you give the engine "shots" of gas because the belt is either gonna come off on accel or decel, just line yourself up with the acc. drive and look at the belt departure angles on every pulley (try both-car running and off), you can see there will be more gap/angle on 1 or more which would likely be the problem. How 'bout a pic? (if you can fit a camera).
